Sports Registration & Physical (required)
These forms are required for ALL athletes. 1. Please download the forms. 2. Fill out all information, make sure both the student and parent have signed each form. 3. See a physician for a physical, and bring the paperwork with you,so that they can fill it out. Each athlete must have an updated physical each year to participate in athletics.

This booklet has been put together by the Ace's Athletic Department to cut through the hype and myths about college athletics recruiting and to help give you an inside track to getting recruited. This booklet does not guarantee a scholarship but does provide valuable information and help for the recruiting process.
